Doula Lab Birth Worker Request Form

The purpose of this form is for birth workers who are interested in using the Doula Lab for classes, meetings, workshops, seminars and more. Please fill out this Google form and allow us up to one week from date of submission to respond and assist with getting you scheduled. We ask that you are booking at least two weeks in prior to your date of need. We are booking May 2024- Nov 2024. There is a fee included in renting the space which will be sent you via email based on your request. We are happy to have you and hope we can help facilitate your needs and the needs of the community. Thank you for your time.
